Valley Medical and Wellness is a proud member of the Fairview Physician Associates Network (FPAN). The Fairview network includes more than 300 primary and speciality care clinics with more than 4,000 physicians and advanced practice providers. In addition to independent clinics, such as Valley Medical, the network includes providers from the M Health Fairview system.
A physician-led non-profit healthcare network, FPAN was created to serve patients in and around the Twin Cities metropolitan area. It has been building relationships between practices and patients for more than 30 years.
FPAN brings healthcare providers together to deliver efficient, affordable, coordinated care to patients. The network cultivates relationships, champions alignment, and fosters payment reform.
Valley Medical works with other members of FPAN to coordinate patient care across clinics and providers. Through FPAN, we can offer our patients easy access to other types of care they may need.
